When you visit your dentist, office staff can usually submit a claim on your behalf to help cover your visit and the care you’ve received. Most policyholders choose to have the benefits paid directly to the dentist.

Unum Dental members can visit any dentist and receive coverage. Whether you use an in-network or out-of-network provider, the same benefit amounts are paid. However, the services are discounted when you use a provider in our national network.

To find a new dentist or check if your existing provider is part of our network, select the card above that matches yours. You’ll then be taken to the correct coverage portal where you’ll be able to search for providers in your area.

We encourage you to receive a pre-treatment estimate on services or treatments that cost more than $300. Although not required, it will provide you with an estimate of benefit coverage and your out-of-pocket expenses.

Dental is not portable, but COBRA coverage is typically available depending on group size and reason for termination.
